Ensure you're on a fast network. Ads are downloaded while you play, so if you have a slow connection, they fail to load and don't show up. Alternatively, you may be using a VPN and ads may refuse to load on hidden connections. I tested it right now on a normal wifi connection and they showed...

I think it's a problem with the counter, as you can't revive three times. It's frozen due the match being lost at server level, I guess. Try restarting the game, it should let you start from room 1.
